Terms and Conditions *

I am the parent or legal guardian of the above-named child. I hereby give my consent for my child to participate in the Aeronautical School of the West Indies Summer Program field trips. I understand that these trips may involve travel to and from various destinations and may include a range of activities related to aeronautics and aviation. I acknowledge and agree to the following: 1. I authorize the Aeronautical School of the West Indies and its representatives to transport my child to and from the designated field trip destinations. 2. In the event of an emergency, I authorize the staff of the Aeronautical School of the West Indies to obtain medical treatment for my child, including hospitalization, anesthesia, surgery, or other medical procedures deemed necessary by a licensed physician. I understand that I will be responsible for any medical expenses incurred. 3. I understand that participation in the field trips involves inherent risks and dangers, including but not limited to transportation accidents, injuries, and other unforeseen events. I voluntarily assume all risks associated with my child’s participation in these field trips. 4. I release and discharge the Aeronautical School of the West Indies, its officers, employees, agents, and volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage sustained by my child during or as a result of the field trips. 5. I understand that my child is expected to follow all rules and instructions given by the staff of the Aeronautical School of the West Indies and to conduct themselves in a responsible and respectful manner. I acknowledge that any misconduct may result in my child being sent home at my expense. 6. I grant permission for the Aeronautical School of the West Indies to take photographs or videos of my child during the field trips for promotional or educational purposes. I understand that these images may be used in printed or electronic media.
